Health Care Employee Assistance
The Health Care Employee Assistance Program (HC EAP) is the newest program within Catholic Charities and continues our mission of service to our brothers and sisters in need. HC EAP assists health care employees who are struggling to make ends meet - those who, for one reason or another, have come upon hard times.
If you are a Providence employee and would like to apply to the Helping Hand Fund, please click on the link below to fill out the application form. After you have filled out the application form, please save it on your computer (titling it with your last name) and then attach it to an email sent to providence@catholiccharitiesoregon.org.
Alternatively, you may fax the form to us at 503-238-4716 or send via regular mail to Catholic Charities, 2740 SE Powell Blvd. #5, Portland, OR 97202.
The Helping Hand Fund is funded exclusively by Providence employees.
After submitting the Helping Hand Fund application form, you may call our toll free number 1-866-996-0334 to discuss your situation.
We will be in touch with you within 48 business hours of receipt of your application.
